Welcome to GRACE. As you can see from this podcast by Dr. Heather Wakelee, the thinking hasn't changed. For stage Ia, it's still unclear that adjuvant chemotherapy provides a benefit, and therefore it's not generally recommended. Of course, treatment decisions can always be individualized when circumstances or patient preference warrants.

Thank you for responding...............also wondering about 1b.........this was the last statement in the report plus all of the doubt throughout the report...............Although some will argue in favor of chemotherapy for certain subsets of IB NSCLC, there is no definitive prospective evidence that patients derive benefit from chemotherapy. Ultimately, patients must discuss the pros and cons of their individual situation with their oncologist

Yes, that's a pretty fair statement of the current thinking about adjuvant chemo for stage Ib. There isn't clear evidence of benefit, so each patient, after discussion with his/her doctor must weigh the general risks and side effects of any chemotherapy regimen against an unknown possible benefit. Some patients will want to be aggressive with treatment; the decision should only be made with a clear understanding of the risks/benefits.
