Has Nivolumab, or another checkpoint inhibitor ever been reported to work on a patient who is apparently having both progression and persistent pneumonia, and is in a situation where it is hard to tell one from the other due to cloudiness in the CT scan?

Welcome to GRACE. According to the Opdivo prescribing information which can be found here a small number of patients taking Opdivo develop pneumonia. Whether Opdivo has caused the pneumonia, and whether it is effectively treating the cancer, would be a question for the patient's local doctors. If Opdivo is causing the persistent pneumonia, that may be an unacceptable side effect requiring discontinuation of the drug, and it may be that the only way to find out is to hold Opdivo for a period to see if the pneumonia improves and does not recur...
As you've probably been told or read, it can take several weeks for the effects of Opdivo to become apparent, although the cloudy scan may prevent that until the pneumonia has cleared.
