It would be very helpful to have some additional background information. I have seen pleural thickening of this size. The pleura itself is a continuous sheath that surrounds the lung. I suspect the 5.2 cm describes the dimension of the sheat that is thickened and the 1 cm is the actual thickness of the sheath. In general, the size of a lesion on a scan is not the most important factor that I look at. Things that I would be more concerning to me would be whether growth was occurring, the rate of growth, whether growth was causing symptoms, etc.
The pleura can sometimes be tricky to measure and sometimes you may be looking at/measuring an area that includes thickening and some fluid.
