Hi,
My boyfriend has been confirmed by biospy to have EGFR L858R mutation. Recently, he did a urine test and the KRAS mutation was detected. Is this possible? I thought KRAS and EGFR were mutually exclusive where you can have one or the other, but not both. What should be the next plan of action? He is currently taking Tarceva with a 60% shrinkage in tumor. His doctor is recommending chemo, but have not recommend what type. What is the best chemo for KRAS? Thank you for your help.

Although it's unusual to have both these mutations, it's not unheard of. What's difficult to know is what percentage of the cancer has the EGFR mutation versus the KRAS. But since Tarceva treatment has resulted in 60% shrinkage to this point, it seems that staying with it makes a great deal of sense, especially if there is no evidence of progression. The general rule is that oncologists tend to prefer getting the maximum benefit from each therapy before discarding it, even to the point of continuing in the face of slow progression, as the alternative may not be as effective.
