Taxotere with no pre-meds? - 1254791

My husband started with stage 3 laryngeal cancer Nov 2010. He was successfully treated with cisplatin and radiation. Recent scans have shown no evidence of disease. Dec 2012 a CT was done and his doc said new growth in the lungs, but didnt appear to be cancer. Because he was drowning in his own fluids, a bronchoscopy was done and it revealed stage 4 NSCLC in both lungs. He also had pseudonymous and micro-bacterium infections. He has done 15 months of 4 different antibiotics for these infections. Visited a surgeon and a radiologist and both were ruled out. With location of the cancer and previous radiation to the brachia plexus radiologist determined that another radiation would be too risky. Started treatment with Carboplatin/Taxol. When the next PET showed mets to lymph nodes & diaphragm treatment was changed to Navelbine. Side effects have been horrible. He has 2 DVT's in his right leg. Swelling of both feet, and now severe neuropathy of right foot. He barely can walk. He also has lost the use of pinky & ring fingers of both hands. He is on warfarin, and his PT INR ranges anywhere from 1.6 to 8. Weekly adjustments of the warfarin have been necessary. Brain MRI and bone scan both been done and have come back clear. After 7 cycles of the Navelbine, a recent PET has shown that the cancer that had been reduced has now become active again in addition he has new mets to another bank of lymph nodes, more growth in the lower lobe of his right lung and now a 2cm lesion in the right lobe of his liver. He still has the effects of the Navelbine, severe neuropathy, swelling, tingling, numbness in both hands & feet. He takes Gabapentin,vicodin,Ativan, Valsartin, iron,ambien,Symbicort and Pro-air. He STILL is working full time. His onco wants to start a new therapy with Taxotere but does NOT want to prescribe and steroid pre-med. Is this really a good idea considering the effects of the Taxotere and the problems my husband already has from treatment

In a previous discussion, Dr.West stated:

"While we tend to be somewhat liberal about omitting steroids or making changes as a premedication for Alimta (pemetrexed), I and just about every oncologist I know prescribe decadron exactly as is recommended for Taxotere (docetaxel). It's reasonable to talk with your oncologist about this difference between what was prescribed and what is commonly recommended."

There may be good reasons why your oncologist doesn't want to prescribe the pre-meds, but you'd need to discuss that with him.

Thanks for your reply. Part of the problem is I have already asked the question, WHY no decadron and his answer was simply because we should try it without first. Considering he already has so much adema & neuropathy, I don't believe that choice is in my husband's best interest. Treatment is supposed to start on 3/20. I will be on the phone with the doc on 3/18 to get answers! Thanks again!

I don't have anything further to add to my prior comments that Jim unearthed for you. I also think there's always room for individual physician judgment, but the general practice is to give premedications before Taxotere, particularly if it's the first time it is being given, as hypersensitivity reactions definitely can occur.
