I don't quite understand your question. If you're asking about whether surgery has a role for stage IV NSCLC, the short answer is no. There is no clear role for it, nor is there a compelling reason to think it would be helpful for the vast majority of people with stage IV NSCLC.
If you're asking about him sleeping non-stop, I don't have an explanation, even though I agree it's quite concerning, partly because I don't know nearly enough about him to speculate about what might be going on. But if you're concerned about him not doing well, this only underscores to me that surgery would be an inappropriate to pursue in this setting.
