Identifying lymph node involvement with VATs surgery - 1295137

Forums

After VATs wedge surgery for a 1a Squamous nodule in my RLL, the oncology surgeon said that he couldn't find any lymph nodes to biopsy. The pathology report said the tumor abuts the pleura, but doesn't infiltrate into it. Surgical resection margins were free of tumor. The tumor had been poorly differentiated, and there was evidence of lympho-vascular invasion.

Silibinin in combination with Tarceva - 1295091

Forums

Dear Moderators and Doctors

I’ve read number of exiting articles published by Spanish group about Silibinin, which is extract of milk thistle. Those papers proved that Silibinin can prolong time to Tarceva resistance and their recent publications proved that Silibinin reduces Brain Mets in lung cancer if used concurrently with systematic treatment.

I am currently on Tarceva for two months and really worry about possibility of brain mets. I would like to try Silibinin, but worry that it could interfere with Tarceva in negative way.

May I please ask you to advise !

An FDA press release states that:

"On August 16, 2018, the Food and Drug Administration granted accelerated approval to nivolumab (Opdivo, Bristol-Myers Squibb Company Inc.) for patients with metastatic small cell lung cancer (SCLC) with progression after platinum-based chemotherapy and at least one other line of therapy."

Opdivo - 1295068

I was reading the article by Dr West, How long should we continue immune checkpoint inhibitor therapy in patients who respond?

This was written in 2016 so I wondered if there is any more info now?

My partner has been on opdivo treatment for 3b nsclc for a little over a year. It’s worked well with all lung tumours stable. He has had lymph node increases, but the oncologist didn’t seem worried.

Several subcentimeter bone lesions; when do you biopsy and/or treat? - 1295059

Dr. West or other oncologist:

I am a 60 yo, post-14 yr stage 2 breast cancer patient. I went to my oncologist in March with a complaint of an off and on burning sensation across the front of my chest and down BOTH arms (feeling like a bad sunburn). All of this has since resolved, except now I have a small area in my thoracic spine that when pressed on, presents a slight burning pain.

Pneumonitis ROS1 - 1295034

Forums

Hoping I may get some answers as it seems everything we try is failing for one reason or another.
Stage 4 ROS1 diagnosed Feb 2018 with BM’s treated with SRS. Ceritinib worked so well apart from pneumonitis so had to stop. Carb and Pem but new BMs just treated again with SRS. Lorlatinib and Entrectinib trials excluded because of pneumonitis!
Having to use Crizotinib and hope now.
Have high PDL1 but prob not helpful. Is there a trial in UK sim to Extended access for Entrectinib?
Any experience using TKI with low dose steroid to control the pneumonitis?
