HELP! Mom diagnosed with leptomeningeal disease after one year on Tagrisso.

My mom has been on her second line treatment, Tagrisso, for over a year. Recently we got a PET/CT scan and a MRI. The PET/CT showed mostly stable disease but two lymph nodes and a bone spot that are showing activity again. The MRI (brain scan) showed 3 suspicious lesions which will be radiated with cyberkife. The problem is, the MRI also showed early signs of leptomeningeal disease (LMD). Obviously this is a very serious situation. Since my mom already had whole brain radiation two years ago, it is not an option. Cyberknife is also not an option for LMD either.

Many types of treatments have been FDA approved for lung cancer that focused on different approaches.  For examples:  Targeted therapies for Alk and EGFR/T790 mutations that targeted specific molecules on the cancer cell; Keytruda focusing on removing cloaks of cancer cells to the immune system; Ketruda plus two other chemos, etc.  Targeted therapy has also been re-introduced after the progression on the latest therapy.  If those therapies worked for an individual, is there any reason why the treatments process cannot just continuously recycle the previous

Treatment with Scleroderma

My sister-in-law was diagnosed 10 years ago with stage 4 adenocarcinoma, with what now would be considered oligiomestastic disease. She had 6 rounds of Cisplatin/Alimta/Avastin followed by approximately 4 years of maintenance chemo (Alimta/Avastin). About 6 years ago they did a lobectomy. Her scans have been clear until a few weeks ago when they saw a 2.1 cm nodule and some ground glass opacity. She is now awaiting biopsy results.

Mixed Results from Treatment

Has anyone had mixed results from chemo/immunotherapy?  My uncle was diagnosed with stage iv nsclc (exon 20 mutation).  It has spread to the bones, lymph nodes and adrenal glands.  He did 6 rounds of chemo with immunotherapy (alimta/keytruda/carbo) and then continued with keytruda/alimta.  We just received his first PET and brain MRI results since his initial diagnosis and while some of the cancer shrunk, some grew and there are new lesions not seen in April.

Local therapies for metastatic NSCLC while on Osmirtinib

Hi. I listened to a panel discussion on Saturday Nov 10, led by Dr Jack West, on local therapies for oligo-progression or oligo-metastatic disease when a systemic therapy like Tagrisso is on place and working. The panel talked about radiation and surgery options. They did not mention internal radiation treatments like SIRT radio-embolization or radio-frequency ablation.

I am in exactly the position described and I will most likely have one of these procedures on my liver shortly.
